Longstanding Health Condition Survey

BHI and the NSW Ministry of Health System Performance Support Branch partnered to develop a survey asking people supported through the NSW Integrated Care Program about their experiences of care.

The Longstanding Health Condition Survey asks people with these conditions to share their experiences of healthcare services in NSW.

Longstanding health conditions last months or years, and include chronic conditions such as diabetes, chronic heart disease, arthritis and severe back pain. Because of the enduring and often complex nature of their illnesses, people with longstanding health conditions have more contact with healthcare professionals and services than those without.

Information collected by the survey will support NSW Health’s work to improve the care these patients receive, particularly how different healthcare professionals and services work together.

Around 21,000 people, identified as receiving integrated care, with a longstanding health condition received a questionnaire between June and August 2019.
