Exploring clinical variation in readmission - Return to acute care following discharge from hospital, eight clinical conditions, NSW, July 2012 – June 2015

This report presents risk-standardised readmission rates for eight clinical conditions in NSW, and across 79 public hospitals. Measures of unplanned readmission are used to inform assessments of healthcare performance and highlight areas for improvement. However, not all readmissions can, or should, be avoided. Some occur when patients develop new health problems, and others are prompted by an acute deterioration in a longstanding chronic condition despite appropriate management. In many cases however, readmission is a poor outcome for patients.
