Longstanding Health Condition Survey

This survey asks people with longstanding health conditions to share their experiences of healthcare services in NSW.

Longstanding health conditions last months or years, and include chronic conditions such as diabetes, chronic heart disease, arthritis and severe back pain. Because of the enduring and often complex nature of their illnesses, people with longstanding health conditions have more contact with healthcare professionals and services than those without.

Information collected by the survey supports NSW Health’s work to improve the care these patients receive, particularly how different healthcare professionals and services work together.
